このページは支部研究大会参加＆宿泊予約および懇親会参加申込ページです。
２０１０年６月５日（土）に開催されます第４０回LET九州・沖縄支部研究大会の参加申込および宿泊等の申込を
このページで行って参ります。

一端、5月20日でまとめさせていただきましたが、参加申込、宿泊予約を再開いたします。
締め切りに間に合わなかった会員・参加希望者の方も忘れずに申込登録をお願いいたします。



　※なお、事前に本部ホームページ（会員登録ページ）（https://www.j-let.org/~honbu/reg_select.php）にて
　　会員登録いただくと、様々な情報が得られますこの機会に入会をお勧めいたします。（年会費6,000円）
　　また、当日、年会費6，000円をお支払いの上、LET会員登録をされますと会員扱いになります。

［注意］
１.　ハウステンボス内の入場料および宿泊（ホテルアムステルダムおよびフォレストヴィラ）の料金は
　　　LET九州・沖縄支部会員（当日会員も含む）のための特別料金となります。
　　　したがって、このフォームからの申込が前提になります。
　　　直接ハウステンボスにご連絡いただいた場合には、特別料金の優遇は受けられません。
　　　ハウステンボスにこの宿泊についての問い合わせはなさらないようお願い申し上げます。
２.　申込フォームで同伴者のお名前をお書きいただいても、他の参加者の登録は必要です。
　　　同室者の確認のためにそれぞれの参加者が同室者のお名前をお書きいただきますようお願い申し上げます。

　　　ただし、メールアドレスが重複すると受け付けない仕様になっていますので、特にご家族の方の場合は、
　　　主たる申込者が代表で同じメールアドレスを使おうとするとうまく登録できません。
　　　同室者がもっておられる携帯メールアドレスを利用して、メールアドレスを変えて登録してください。
　　　携帯に送られた情報はそのままでは印字できませんので、受付時に携帯の内容を提示していただくか、
　　　あるいは、携帯メールをPCメールに転送して印字した上でご持参ください。

３.　このフォームでは金額の自動計算はおこなえません。
　　　したがって、皆様が内容をよくご覧になって、間違いなく選択していただくことが前提になります。
　　　お手数をおかけしますが、よろしくお願いします。
４.　予約登録完了の際に送信されるメールをプリントアウトして、必ず研究大会当日に持参してください。
５.　お支払いにつきましては、以下の通りです。
　　(a) 懇親会費と当日会費は、学会会場の受付にて現金でお支払い下さい。
　　(b) 宿泊代のお支払いは、それぞれの宿泊施設でチェックアウト時にお願いします。

（１）参加費・宿泊等に関する情報は以下の通りです。

【参加費用・入場料】
1) ハウステンボス内に学会割引料金で宿泊される場合：
・会員：無料
・当日会員：参加費1,000円（資料代を含む）のみ


2) ハウステンボス内に学会割引料金で宿泊されない場合：
・会員：入場料1,000円のみ
・会員で既に入場チケットをお持ちの方：無料
・当日会員：参加費1,000円（資料代を含む）＋入場料1,000円＝合計2,000円
・当日会員で既に入場チケットをお持ちの方：参加費1,000円（資料代を含む）のみ

【宿泊費】
1) ホテル・アムステルダム宿泊の場合：
・シングル利用13,965円
・ツイン利用11,340円
・トリプル利用9,975円
・フォース利用8,820円
2) フォレストヴィラ宿泊の場合：
・トリプル利用もフォース利用も10,395円
【キャンセル料】
1) 不泊100％
2) 当日80％
3) 前日20％

（２）懇親会について
懇親会：フォレストビラ　レストラン「トロティネ"Trottiner"」
　　　　参加費各自6,000円（フリードリンク代は支部負担；LET会員特別料金です。）

because you will get more useful information from LET. Annual Membership Fee is 6,000 yen.

You can register your membership at the reception desk on the day of the conference
and you will be treated as an LET member.
But we strongly recommend the advanced LET membership registration for you.

Notanda:
1. Special Fees apply only to the members who registered the participation and accommodation FROM THIS PAGE.
Please do not get direct contact with Huis ten Bosch.
2. Each participant has to fill the names of his/her chamber fellows.
And each participant has to register individually, even if you fill in the names of your chamber fellows.

Please note, however, that chamber fellows don't use the same email address with main registerer.
Especially, when your chamber fellows are your family members, you may try to use your email address
for your chamber fellows. This won't work well. You have to use another email address, for example,
cell phone's email address. It works well. However, you cannot print out reservation information.
At the reception counter, your chamber fellow will show the email of the cell phone,
or by forwarding the cell phone email to your PC mail and you print out the forward mail content.
Then bring the printed paper.
　
3. This form system cannot sum up automatically. You have to make sure of your choices.
4.　Please print out the mail content that is going to be sent to you when the reservation is completed,
and never fail to bring it on the day of the conference.
5. As to payment, we would like to ask you to pay for (a) the party fee and one-day-member fee in cash at the reception desk on the day of the conference, (b) your accommodation fee at your check-out time at each accommodation.

(1) Information of Participation and Accommodation
[Participation Fee & Admission charge]
1) When you stay in Huis ten Bosch with special discount charge for LET members:
・LET Members: Free of charge
・One Day members: \1,000 (including reference materials)

2) When you do not stay in Huis ten Bosch:
・LET Members: \1,000 for Admission charge for Huis ten Bosch.
・LET Members and you already have an admission ticket: Free of charge
・One Day members: Total \2,000: Participation Fee \1,000 (including reference materials)＋Admission charge \1,000
・One Day members and you already have an admission ticket: Participation Fee \1,000 (including reference materials)

[Accommodation Fee]
1) In case of staying at Hotel Amsterdam:
・Single use : \13,965
・Twin use (two people in one room): \11,340
・Triple use (three people in one room): \9,975
・Fourth use (four people in one room): \8,820
2) In case of staying at Forest Villa:
・Triple & Fourth use: \10,395
[Cancel Charge]
1) Not staying: 100％
2) On the day: 80％
3) Preceding Day: 20％

(2) Information of Reception Party
Place: Restaurant "Trottiner" at Forest Villa
Participation fee: \6,000 (Special Dinner at a charge of the special discount for the LET members.)
(The fee for drinking is borne by LET-KO chapter.)

予約登録をする際には以下のことをご注意ください。
（１）赤の＊印は必須項目です。必ず記入をお願いします。
（２）「予約を申し込む」ボタンを押した後、確認画面が出ますので、
　　　ここで「確認して送信」ボタンを必ず押してください。
　　　つまり、２画面でそれぞれ１回ずつ（計２回）ボタンを押すことになります。
（３）記入されたメールアドレスに確認のメールが届きましたら、
　　　予約登録が完了になります。

　　　つまり、２画面でそれぞれ１回ずつ（計２回）ボタンを押すことになります。
（３）２回のクリックが完了したら、登録の完了画面になります。この時点でメールが自動で配信されます。
（４）記入されたメールアドレスに確認のメールが届きましたら、
　　　予約登録が完了になります。

「予約を申し込む」ボタンを押しただけでは予約になりません。ご注意ください。

1. Items with asterisk are required.
2. After clicking "Go to Confirm" button, Confirmation page will appear. Then surely click "Reservation" button. With one click in each page (with total two clicks), your reservation will be confirmed.
3. After clicking "Reservation" button, "Your reservation completed" page appears and at the same time completion mail will automatically be sent to your mail account.
4. You receive a reservation mail, then your reservation completes.
Only clicking "Go to Confirm" button will not complete your reservation process.
